The award was established in memory of Diane to recognize those who exemplify the same qualities she possessed: active acceptance, respect, compassion, devotion, and caring for people with disabilities. Nominees may also include volunteers, organizations or programs that have established advocacy services and/or increased community resources for people with disabilities.
Recipient(s) will be announced in early February and honored at the annual luncheon of the Arizona Chapter, Branch 2 of the National Association of Social Workers in March 2018. A plaque and cash award will be presented at the luncheon.
The deadline for nominations is Friday, January 19, 2018 by 5:00 p.m.
Nominations are submitted through the Common Grant Application. A profile must be created in order to submit a nomination. If you have previously used the Common Grant Application to submit a grant request to CFSA, you may use your existing login. Click here for a brief tutorial on using the Common Grant Application.
